Care Assessment

Though you may not be your loved one’s primary caregiver, you are still a vital part of the care team that is involved in your loved one’s life. Each resident and family will meet with various departments and staff members to help determine the personalized care plan for your loved one.

Reserve a Suite

Once you have decided that Cross Creek is the best place for your loved one, complete a reservation agreement and pay a one-time, nonrefundable reservation and community fee of $2,000.

We understand that it can also be filled with emotional challenges that require significant preparation and planning. Our team is ready to help make the transition as smooth as possible for our residents and their families.

Preparing the Suite

Our staff will offer advice in an effort to help your loved one be informed and involved in the move ahead. Suites are unfurnished to allow residents and their families to fill their new spaces with beloved items and furniture to make them feel at home. Prior to your loved one’s move into Cross Creek, our staff will work with family members to set up the suite so that when your loved one arrives to Cross Creek, they arrive home.

A Transition Checklist

Our skilled care team members are involved in each resident’s move and have gained expert knowledge to make your move as smooth as possible. Speak with our team to determine what items you can bring, what to leave behind and to receive helpful advice on the transition process.

Personalized Decor

Creating a personalized environment that feels like home with photos, mementos and other familiar items will help your loved one keep their sense of identity and belonging.
